A serious incident
Creating a system of transparency is vital to ensuring lessons can be learnt from the most serious of incidents that unfortunately take place in hospitals.
When such variation exists across the NHS, as revealed in the new BBC Scotland investigation, it makes it far more difficult to grasp the true scale of the incidents, and what the best approach to improvement could be.
Better reporting, a standardised level of what is considered serious, and timely responses could help to drastically improve patient care and safety.
Getting a clearer picture of what is really going on is the first step to recovery.
